We are currently seeking an experienced Physical Therapist to perform home health visits for our senior living office in Blue Bell, PA. This role will involve working with adult and geriatric clients on a per visit basis in territories throughout Montgomery County.
As a home care Physical Therapist, you will be part of a multidisciplinary team providing rehabilitative care and skilled nursing services to clients, enabling them to receive the necessary medical care to remain at home.
Responsibilities:
- Conduct home visits to clients in designated geographic areas.
- Perform diagnostic tests and measurements, such as mobility, range of joints, transfer status, stability, ambulation patterns, strength, endurance, balance testing, and safety assessments.
- Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continuously assess and revise the Physical Therapy care plan, and participate in the multidisciplinary care plan as needed.
- Educate and instruct clients, family members, or representatives on rehabilitative care and activities necessary to promote their health, safety, and independent living.
- Accurately document observations, interventions, and evaluations related to client care management and services provided using a state-of-the-art touch pad tablet.
Qualifications:
- Pennsylvania Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
