Job Description
As a Home Health Physical Therapist, you will play a vital role in providing rehabilitative care and skilled nursing to clients in their homes. You will be an integral member of a multi-disciplinary healthcare team, working collaboratively with nurses, social workers, and other healthcare professionals to develop and implement personalized care plans.
Your responsibilities will include:
- Conducting home visits to assess clients' physical therapy needs and develop individualized care plans.
- Performing diagnostic tests and measurements to evaluate clients' range of motion, strength, and mobility.
- Implementing manual therapeutic exercises, gait training, and other interventions to promote clients' recovery and independence.
- Educating clients and their families on proper techniques for rehabilitation and promoting healthy living habits.
- Accurately documenting client assessments, interventions, and progress in electronic medical records.
Requirements
To be successful as a Home Health Physical Therapist at BAYADA, you will need:
- A current Pennsylvania Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively in a fast-paced environment.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills and experience with electronic medical records (EMR) preferred.
