The responsibilities of this role include:
- Conducting home visits to clients in designated areas.
- Performing diagnostic tests and measurements, such as assessing mobility, range of joints, transfer status, balance testing, and safety assessments.
- Implementing interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assessing and revising the physical therapy care plan and participating in multidisciplinary care planning as necessary.
- Educating and instructing clients, family members, or other representatives on rehabilitative care and activities necessary for health, safety, and independent living.
- Accurately documenting observations, interventions, and evaluations related to client care management and services provided.
