Responsibilities
Making home visits to clients in designated geographic territories, performing diagnostic tests and measurements, developing and implementing individualized care plans, educating and instructing clients, family members, or other representatives, and accurately documenting observations, interventions, and evaluations.
Qualifications
- A current Physical Therapist license in the state
- A minimum of one year of recent work experience as a Physical Therapist
- Previous home health care experience welcomed, but not required
- Solid computer skills and prior experience with electronic medical records (EMR)
