BAYADA Home Health Care is currently seeking a part-time or full-time Outpatient Physical Therapist, PT, for performing outpatient visits in independent living/assisted living/memory care communities in the Fort Collins area.
As an outpatient Physical Therapist, you will be an integral member of a multi-disciplinary healthcare continuum team that provides outpatient services to clients affording them the opportunity to age in place.

Responsibilities:
- Make outpatient visits to clients in designated senior living communities in the Loveland area.
- Supervise Physical Therapy Assistants as necessary or applicable, in accordance with state licensure requirements and professional standards.
- Attend and contribute to patient care, staffing conferences and other related meetings.
- Makes referrals to other disciplines in order to address the comprehensive needs of the patient.
- Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, patterns and appearance of ambulation, strength and endurance of muscles, balance testing, and safety assessments.
- Develop and implement appropriate individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
- Continually assess and revise the Physical Therapy - PT plan of care, and participate in the multidisciplinary care plan meetings including community staff, as appropriate.
- Educate and instruct clients, family members, or community staff, in therapy services and activities necessary to promote the client's health, safety, and independent living.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet.
Qualifications:
- A current CO Physical Therapist - PT license.
- Outpatient experience a plus but not required.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association, as indicated by school transcript or diploma.
- Ability to work independently and manage time effectively.
- Dynamic and strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills: prior experience with electronic medical records (EMR) preferred.
