BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team in Yadkinville, North Carolina.
The successful candidate will provide high-quality rehabilitative care to adult and geriatric clients in their homes, working independently and as part of a multidisciplinary healthcare team.
Responsibilities
- Make home visits to clients in designated territories.
- Perform diagnostic tests and measurements to assess client needs.
- Implement individualized care plans, including manual therapeutic exercises and gait training.
- Educate clients and family members on rehabilitative care and activities to promote independent living.
- Document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
Qualifications
- A current North Carolina Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
