About the Role:
We are seeking a skilled and compassionate Home Health Physical Therapist to join our team in Woburn, Massachusetts.
Key Responsibilities:
- Perform diagnostic tests and measurements, such as mobility and strength assessments, balance testing, and safety evaluations.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
- Continually assess and revise the Physical Therapy care plan, participating with nursing in the multidisciplinary care plan as needed.
- Educate and instruct clients, family members, or representatives on rehabilitative care and activities promoting health, safety, and independent living.
Requirements:
- A current license as a Physical Therapist in Massachusetts is required.
