Responsibilities
- Making home visits to assess and treat patients with physical impairments or disabilities
- Developing and implementing individualized care plans to promote patient independence and mobility
- Performing diagnostic tests and measurements to assess patient progress and adjust treatment plans as needed
- Educating patients, family members, or caregivers on rehabilitative care and activities to promote health, safety, and independent living
- Accurately documenting patient observations, interventions, and evaluations using electronic documentation systems
Requirements
- Current license as a Physical Therapist in Massachusetts
- Experience working with adult and geriatric populations in home health settings preferred
