Home Health Physical Therapist

About the Role

We are seeking an experienced Physical Therapist to join our team in Plymouth, New Hampshire. The ideal candidate will have a strong clinical background and excellent communication skills.

Responsibilities

• Conduct home visits to clients in designated territories to provide rehabilitative care and skilled nursing services.
• Perform diagnostic tests and measurements to assess client mobility, strength, and balance.
• Develop and implement individualized care plans to promote client health, safety, and independent living.
• Educate and instruct clients and family members on rehabilitative care and activities necessary for optimal outcomes.
• Accurately document client observations, interventions, and evaluations using state-of-the-art touch pad technology.

Requirements

• A current New Hampshire Physical Therapist license.
• A minimum of one year of recent work experience as a Physical Therapist.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills and prior experience with electronic medical records (EMR) preferred.
