Job Description
We are seeking a skilled Physical Therapist (PT) to provide home health services to adult and geriatric clients in Guilford and Rockingham counties.
Responsibilities:
- Make home visits to clients in designated territories.
- Perform diagnostic tests and measurements, including mobility assessments and safety evaluations.
- Implement protective interventions to prevent wound formation and promote wound repair.
- Develop and implement individualized care plans, including manual therapeutic exercises and gait training.
- Continually assess and revise the PT care plan, and participate in multidisciplinary care planning as needed.
- Educate and instruct clients, family members, or other representatives in rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Requirements:
- A current North Carolina Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records preferred.
