Job Title: Home Health Physical Therapist
BAYADA Home Health Care is seeking a dedicated and compassionate Physical Therapist to join our team in Scotland County, North Carolina.
This opportunity allows you to work as an integral member of a multi-disciplinary healthcare team, providing rehabilitative care and skilled nursing to clients in the comfort of their own homes.
Responsibilities:
- Make home visits to clients in designated geographic territories
- Perform diagnostic tests and measurements
- Develop and implement individualized care plans
- Educate and instruct clients on rehabilitative care and activities necessary for independent living
- Accurately document observations, interventions, and evaluations
Qualifications:
- A current North Carolina Physical Therapist license
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Ability to work independently and manage time effectively
- Strong interpersonal, organizational, and problem-solving skills
- Solid computer skills and prior experience with electronic medical records (EMR)
