About the Role
">- We are seeking a compassionate and dedicated Physical Therapist Assistant (PTA) to join our team in Cape May Courthouse, NJ office.
- This role will involve making home visits to adult and geriatric clients on a per visit basis in territories throughout Cape May county.
- The ideal candidate will have a strong passion for delivering quality 1:1 care to clients while maintaining work-life balance.
Responsibilities
">- Follow a designated care plan in accordance with patient's needs.
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
- Monitor clients' conditions; reporting changes to Clinical or Client Services Manager.
- Case management and coordination.
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided.
Requirements
">- A current license as a Registered Nurse in New Jersey is required.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience is necessary.
- Prior home care experience is strongly preferred but not required.
- Graduation from an accredited and approved nursing program is essential.
