Responsibilities

- Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions between healthcare settings.

Requirements
- Certification or licensure as a Physical Therapist Assistant (PTA) in the state of Vermont.
- Graduation from an accredited Physical Therapist Assistant program.
- At least one year of experience working as a Physical Therapist Assistant in a home health or related setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations and healthcare professionals.
