Responsibilities:
- Conduct thorough assessments of patients' needs and develop personalized care plans to promote optimal health outcomes.
- Deliver high-quality patient care in a variety of settings, including hospitals, clinics, and patients' homes.
- Collaborate with interdisciplinary teams to ensure seamless communication and coordination of care.
- Document all patient interactions and progress in accordance with company policies.
Requirements:
- A current license as a Registered Nurse in Pennsylvania.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience strongly preferred, but not required.
