Job Description
As a Home Health Rehabilitation Therapist, you will be responsible for providing high-quality care to clients in their homes. This role requires a licensed Physical Therapist with one year of prior clinical experience to assess clients' physical abilities, develop individualized care plans, and provide education and training to clients and their families.
Responsibilities
- Conduct thorough assessments of clients' physical abilities and create personalized care plans.
- Develop and implement effective interventions to improve clients' mobility, strength, and overall quality of life.
- Collaborate with nursing and other healthcare professionals to ensure comprehensive care.
- Accurately document client progress and services provided.
Requirements
- A current PA Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills; prior experience with electronic medical records (EMR) preferred.
