About the Role: BAYADA Home Health Care seeks a skilled Home Health Therapist to provide exceptional care and support to our clients in Rutland, Vermont. As an integral member of our multidisciplinary healthcare team, you will be responsible for delivering rehabilitative care and skilled nursing services to clients in their homes, empowering them to maintain independence and dignity.
Key Responsibilities:
- Conduct thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Develop individualized intervention plans to achieve favorable outcomes and continuously assess and revise the occupational therapy care plan as needed.
- Educate and instruct patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote the client's health, safety, and independent living.
Qualifications:
- A current Vermont Occupational Therapist license.
- Prior home care experience preferred, but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
