About the Role

As a home health physical therapist, you will be an integral member of a multidisciplinary healthcare team providing skilled nursing and rehabilitative care to adult and geriatric patients in their homes.
Responsibilities include making visits to clients in their homes, performing diagnostic tests and measurements, developing and implementing individualized care plans, and educating clients on rehabilitative care and activities necessary for independent living.
