About the Role:

The Physical Therapist will be responsible for conducting home visits, assessments, and admissions for adult and geriatric clients in designated territories throughout Gaston and Cleveland county.

Key Responsibilities:
- Conduct thorough assessments of clients' physical abilities and create individualized care plans to meet their needs.

- Educate clients and their families on rehabilitative care and activities necessary to promote health, safety, and independent living.
Qualifications:
- A current NC Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
