About the Job
BAYADA Home Health Care is seeking a qualified Physical Therapist to join our team in Cumberland County, New Jersey. As a home health Physical Therapist, you will be responsible for providing rehabilitative care and skilled nursing services to clients in their homes.
Key Responsibilities:
- Making home visits to clients in designated territories to assess and address their physical therapy needs.
- Developing and implementing individualized care plans to promote client independence and safety.
- Educating clients and their families on rehabilitation techniques and activities necessary for optimal health and well-being.
- Accurately documenting client observations, interventions, and evaluations using electronic medical records.
Requirements:
- A current NJ Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills and prior experience with electronic medical records (EMR).
