Home Health Therapist Specialist

About the Role: We are seeking a skilled Physical Therapist to join our team at BAYADA Home Health Care. As a Home Health Physical Therapist, you will be responsible for providing high-quality rehabilitative care to clients in their homes.

Key Responsibilities:

• Making home visits to clients in designated geographic territories
• Performing diagnostic tests and measurements
• Developing and implementing individualized care plans
• Educating clients and family members on rehabilitative care and activities
• Accurately documenting client care management and services provided

Requirements:

• A current New Hampshire Physical Therapist license
• A minimum of one year of recent work experience as a Physical Therapist
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
• Ability to work independently and manage time effectively
• Strong interpersonal, organizational, and problem-solving skills
