Job Description:
This position requires a compassionate and skilled Home Health Physical Therapy Assistant to serve patients in Gloucester county. This role involves working collaboratively with a multidisciplinary team to provide comprehensive care.
Responsibilities:
- Provide patient education on self-management techniques and assist with goal setting.
- Conduct ongoing assessments to ensure clients receive the most effective interventions.
- Work collaboratively with nurses and other healthcare professionals to develop individualized care plans.
Qualifications:
- Current licensure or certification in New Jersey.
- Two-year degree from an approved Physical Therapy program.
- At least one year's experience working under the supervision of a licensed Physical Therapist.
- Strong interpersonal and communication skills, along with excellent problem-solving abilities.
