Key Responsibilities
- Deliver high-quality patient care in the comfort of their own homes, working closely with licensed Physical Therapists to develop and implement personalized care plans.
- Assist patients in achieving optimal physical wellbeing through a range of interventions, including treatment, testing, education, and advocacy.
- Continuously assess and revise patient care plans, collaborating with nursing teams and other multidisciplinary stakeholders as needed.
Requirements and Qualifications
- Current licensure or certification in the state of New Jersey
- Graduation from a two-year college level program approved by the American Physical Therapy Association, or relevant experience as a Physical Therapist Assistant
- At least one year's work experience under the supervision of a qualified Physical Therapist
- A calm and reassuring presence, active listening skills, excellent assessment skills, and strong written and verbal communication
- The ability to deliver patient care with compassion, excellence, and reliability
