Job Responsibilities:
- Making home visits to clients in designated geographic territories.
- Performing diagnostic tests and measurements, such as mobility and range of motion assessments, transfer status evaluations, stability testing, and safety assessments.
- Implementing integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
- Developing and implementing individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
Qualifications:
- A current NJ Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
