In-Home Rehabilitation Services - Physical Therapist Wanted

Responsibilities:
- Making in-home visits to assess clients' physical abilities and develop personalized care plans.
- Implementing manual therapeutic exercises and gait training programs to enhance clients' mobility and independence.
- Educating clients and their families on safe rehabilitation practices and activities that promote self-care.
- Accurately documenting client interactions, progress, and treatment outcomes in electronic medical records (EMR).
Requirements:
- A valid Pennsylvania Physical Therapist license.
- At least one year of experience working as a Physical Therapist in a similar setting.
- Degree from an accredited American Physical Therapy Association-approved program.
- Excellent communication and interpersonal skills.
- Proficiency in electronic medical records (EMR) software preferred.
