Responsibilities:
- Visit patients in their homes to provide skilled nursing care, including wound care, medication management, and education on self-care techniques.
- Assess patients' needs and develop individualized care plans to promote optimal health outcomes.
- Collaborate with healthcare teams to ensure seamless communication and coordination of care.
- Document all patient interactions and progress in accordance with company policies.
Requirements:
- A current license as a Registered Nurse in Pennsylvania.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care experience strongly preferred, but not required.
