BAYADA Home Health Care seeks a Physical Therapist for a part-time home health office in Millville, NJ. As a member of a multidisciplinary healthcare team, you will provide rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care at home.
Responsibilities:
- Perform diagnostic tests and measurements, including mobility and strength assessments.
- Develop individualized care plans with manual therapeutic exercises, gait training, balance interventions, and other treatments.
- Continually assess and revise the care plan, participating in multidisciplinary care planning as needed.
- Educate clients and family members on rehabilitative care and activities promoting independent living.
- Accurately document client observations, interventions, and evaluations using electronic medical records.
Qualifications:
- NJ Physical Therapist license.
- Minimum one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Solid computer skills; prior EMR experience preferred.
