Key Responsibilities:
- Develop and implement customized care plans for patients.
- Provide personalized home visits, ensuring clients receive high-quality care and support.
- Administer medications, provide wound care, and perform other assigned duties.
- Monitor patient conditions, reporting changes to Clinical or Client Services Managers.
- Document all actions accurately, following up with necessary steps.
- Conduct regular client assessments, applying expertise in case management and coordination.
Requirements and Qualifications:
- Current RN licensure in NJ.
- At least one year of recent clinical experience.
- Prior home care experience a strong asset but not mandatory.
- Nursing degree from an accredited program.
- Effective time management and strong interpersonal skills.
- Proficiency in using electronic medical records and solid computer skills.
- Ability to travel to various locations as needed.
