Job Description
As a nurse care specialist, you will have the opportunity to deliver skilled nursing, interventions, and supportive care to patients in their homes.
You will work closely with our interdisciplinary team to develop individualized care plans that meet the unique needs of each patient.
- Perform 5-6 visits per day in a small geographical area, focusing on individualized care.
- Deliver skilled nursing, interventions, and supportive care according to individual care plans.
- Conduct client assessments, provide education, and monitor patients' conditions.
Required Skills and Qualifications
To qualify for this role, you must have:
- A current NJ nursing license in good standing.
- One year of clinical experience preferred.
- Previous home healthcare experience is welcome, but not required.
