Job Overview

The successful candidate will have a current license as a Registered Nurse in Pennsylvania and a minimum of one year of recent, verifiable clinical (medical/surgical) experience. Prior home care experience is strongly preferred but not required.

- Assess client needs and develop comprehensive care plans in accordance with patient requirements
- Make home visits to clients in designated geographic territories, providing quality care and support
- Perform assigned duties, including administration of medication, wound care, treatments, and procedures, while ensuring accurate documentation and adherence to doctors' orders
- Monitor clients' conditions, reporting changes to Clinical or Client Services Manager, and facilitate effective case management and coordination
