Job Title: Home Health Registered Nurse

Responsibilities:
- Provide skilled nursing services to clients in their homes, prioritizing patient-centered care.
- Assess patients' conditions, develop and implement individualized care plans, and make adjustments as needed.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Monitor patients' progress, reporting changes to clinical managers and making recommendations for continued care.
Qualifications:
- A current license as a Registered Nurse in North Carolina
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Prior home care experience is highly desirable but not required
- Graduation from an accredited and approved nursing program
- Strong interpersonal skills, with the ability to work independently and manage time effectively
- Solid computer skills, with prior experience using electronic medical records (EMRs)
- Ability to travel to cases as assigned
