The successful candidate will have the opportunity to work with adult and geriatric patients on a per visit basis in homes throughout Lamoille and Orleans Counties.
As a home care Occupational Therapist, you will be an integral member of a multi-disciplinary health care team that provides rehabilitative care and skilled nursing to clients, affording them the opportunity to receive the medical care required to remain at home.
You will be responsible for making home visits to clients, assessing their functional status, developing individualized intervention plans, and educating patients and family members on rehabilitative care and activities necessary to promote independent living.
