<3. Continuously assess and revise the occupational therapy care plan as needed, collaborating with nurses and other healthcare professionals when necessary.
<4. Educate patients, family members, or caregivers on rehabilitative care and activities to promote optimal health and safety.
<5. Accurately document observations, interventions, and evaluations related to patient care management and services provided using state-of-the-art technology.
**Requirements include:**
* Current Vermont Occupational Therapist license
* Prior home care experience preferred but not required
* Minimum one year of recent work experience as an Occupational Therapist
* Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
* Ability to work independently and manage time effectively
* Strong interpersonal skills, organizational skills, and problem-solving skills
* Solid computer skills, with prior experience with electronic medical records (EMR) a plus
