Key Responsibilities
- Evaluate patients' functional status and develop individualized intervention plans to address specific needs and goals.
- Conduct thorough assessments of patients' home environments to identify potential hazards and barriers to independent living.
- Collaborate with other healthcare professionals to develop and implement comprehensive care plans that promote patient safety, comfort, and well-being.
- Communicate effectively with patients, families, and caregivers to ensure seamless care transitions and maximize patient engagement.
Requirements
- Prior experience as an Occupational Therapist in a home healthcare setting or related field preferred.
- Current licensure as an Occupational Therapist (OT) in the state of Pennsylvania required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation (CAHEA).
- Ability to work independently and manage time effectively in a fast-paced environment.
- Strong communication and interpersonal skills to interact with diverse populations.
