BAYADA Home Health Care is seeking a highly skilled and compassionate Occupational Therapist to join our team in Lakemore, Ohio.
About the Role
The successful candidate will be responsible for delivering high-quality outpatient therapy services to adult and geriatric patients in their homes throughout Stark County.
We are looking for an experienced therapist who can work independently and as part of a multidisciplinary team to provide rehabilitative care and skilled nursing to clients, enabling them to receive the medical care required to remain at home.
The ideal candidate will have strong interpersonal skills, excellent communication abilities, and the ability to work effectively with clients, family members, and other healthcare professionals.
- Key Responsibilities:
- Perform outpatient therapy evaluations to develop individualized plans of care for patients
- Deliver high-quality therapy services to maximize functional independence for clients
- Work collaboratively with the home health therapy team to transition care from home health to outpatient services
- Educate clients, family members, and caregivers on care plans to achieve client-centered goals
Requirements
To be considered for this position, you must possess:
- A current OH Occupational Therapist license
- Prior home care experience is preferred but not required
- A minimum of one year of recent work experience as an Occupational Therapist
- Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA
- Ability to work independently and manage time effectively
- Strong interpersonal skills, organizational skills, and problem-solving skills
- Solid computer skills; prior experience with electronic medical records (EMR) is preferred
