BAYADA Home Health Care is a leading provider of home health care services, seeking a highly skilled Physical Therapist to join our team in Sierra Vista. As a member of our multidisciplinary healthcare team, you will provide skilled nursing and rehabilitative care to clients in their homes, enabling them to remain at home.

Your Key Responsibilities

• Conduct diagnostic tests and measurements to assess clients' mobility, strength, balance, and safety.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
• Continually assess and revise the Physical Therapy - PT care plan.
• Educate and instruct clients, family members, or other representatives on rehabilitative care and activities necessary for independent living.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.

Qualifications

• A current Physical Therapist - PT license in Arizona.
• Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
