About the Position

- Makes home visits to clients in designated geographic territories.
- Performs diagnostic tests and measurements to assess client mobility and strength.
- Develops and implements individualized care plans to promote wound repair, balance, and overall physical function.
- Provides education and support to clients and family members on rehabilitative techniques and activities necessary for independent living.
Responsibilities
">- Provides individualized rehabilitation services to adult and geriatric clients in their homes.
- Develops and implements care plans to address client needs and goals.
- Collaborates with multidisciplinary healthcare teams to ensure seamless care delivery.
Requirements and Qualifications
">- A current NJ Physical Therapist license.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong computer skills; prior experience with electronic medical records preferred.
