Key Responsibilities
- Develop and implement personalized care plans in collaboration with clients, family members, and other healthcare professionals.
- Provide skilled nursing care, including administration of medications, wound care, treatments, and procedures.
- Monitor clients' conditions and report any changes to the Clinical Manager and Client Services Manager.
- Document all interactions with clients, including medication administration, treatments, and client responses.
The ideal candidate will possess strong critical thinking skills, a compassionate approach to care, and the ability to work collaboratively with others.
Qualifications
- A current license as a Licensed Practice Nurse in the state of NJ.
- A minimum of one year of recent, verifiable experience as an LPN.
- Graduation from an accredited and approved nursing program.
- Strong computer skills and prior experience with electronic medical records (EMR) preferred.
