- Performing evaluations, treatments, and proper discharge planning for your clients using an outpatient EMR platform.
- Working closely with other home health therapy partners to transition care from home health to outpatient services.
- Educating clients, families, and caregivers on care plans created for your patients.
- Providing rehabilitative care and skilled nursing to adult and geriatric patients in their homes throughout Stark County.
