Job Title: Home Health Physical Therapist
BAYADA Home Health Care is seeking a skilled and compassionate Physical Therapist to join our team in Port Carbon, Pennsylvania.
About the Role:
- As a Home Health Physical Therapist, you will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing services to clients in their homes.
- You will perform diagnostic tests and measurements, develop and implement individualized care plans, and educate clients and family members on necessary activities for independent living.
- Your responsibilities will include assessing and revising care plans, participating in multidisciplinary care planning, and accurately documenting client care management and services provided.
Requirements:
- A current Pennsylvania Physical Therapist license
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred
