Job Details
As a Registered Nurse on our team, you will work closely with Clinical Managers to develop and implement customized care plans that cater to the unique needs of each client. Your expertise will enable you to assess patients' conditions, administer medications, provide wound care, and perform various medical procedures as required.
Responsibilities
- Clinical Care: Deliver high-quality patient care in clients' homes, including medication administration, wound care, and other medical interventions.
- Care Planning: Collaborate with Clinical Managers to develop and implement personalized care plans that meet the unique needs of each client.
- Communication: Effectively communicate with clients, families, and healthcare teams to ensure seamless care coordination.
- Documentation: Accurately document all interactions, observations, and interventions in clients' electronic medical records.
Requirements
- A current license as a Registered Nurse in Pennsylvania is mandatory.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience is required.
- Prior home care experience is strongly preferred but not necessary.
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma, is essential.
- The ability to work independently and manage time effectively is crucial.
- Solid interpersonal skills are necessary for effective communication with clients, families, and healthcare teams.
- Familiarity with electronic medical records (EMR) systems is preferred.
- The ability to travel to cases as assigned is required.
