As a Registered Nurse, you will be responsible for providing comprehensive patient care in clients' homes. This role requires a minimum of one year of recent clinical experience and a valid RN license in Pennsylvania.
Responsibilities:
- Assess and develop personalized care plans in collaboration with clients, families, and healthcare teams.
- Conduct home visits to evaluate clients' medical requirements and administer necessary care.
- Document all interactions, interventions, and evaluations accurately using EMR systems.
- Report any changes in clients' conditions to Clinical or Client Services Manager and follow up with doctor's orders.
Qualifications:
