We are currently seeking an experienced LPN to join our team and provide 12-hour night shifts to a sweet 11-year-old client in Cherry Hill. This client has undergone a heart transplant and requires ongoing trach/vent care and g-tube management. If you have experience working with pediatric clients with complex medical needs, we encourage you to apply.
Responsibilities:
- Providing direct patient care, including tracheostomy and ventilator management
- Developing and implementing individualized care plans
- Collaborating with the multidisciplinary team to ensure seamless care transitions
- Maintaining accurate and detailed documentation of patient care
