Job Responsibilities:
- Conduct home visits to assess patients' functional status and develop personalized treatment plans.
- Implement individualized intervention plans to promote independence and improve quality of life.
- Evaluate home environments for hazards or barriers to independent living and identify equipment needs and environmental adaptations.
- Collaborate with other healthcare professionals to ensure seamless care and achieve optimal patient outcomes.
Required Qualifications:
- A current NC Occupational Therapist license.
- Prior home care experience preferred but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
