Job Overview
BAYADA Home Health Care is seeking a dedicated and compassionate Registered Nurse (RN) to join our team as a Per Diem Nurse - Hospice Care Specialist.
Job Responsibilities
- Making home visits to clients in designated geographic territories to provide palliative care and support.
- Performing assigned duties, including administration of medication, wound care, treatments, and procedures, with a focus on comfort care.
- Monitoring clients' conditions and reporting changes to the Clinical or Client Services Manager.
- FOLLOWING UP WITH, EXECUTING, AND PROPERLY DOCUMENTING DOCTORS' ORDERS.
- Performing client assessments as necessary to ensure optimal care and outcomes.
- OCCASIONAL ON CALL AFTER HOURS AND/OR WEEKENDS
Requirements
- A current license as a Registered Nurse (RN) in New Jersey.
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
- Prior home care and/or hospice experience strongly preferred but not required.
- Solid computer skills and prior experience with electronic medical records (EMR) are desirable.
- Ability to travel to cases as assigned.
