You will be responsible for providing high-quality, patient-centered care to individuals in their homes, helping them achieve their goals and regain independence.
In this role, you will:
- Evaluate patients' functional status and develop individualized intervention plans to achieve favorable outcomes.
- Make home visits to clients in designated geographic territories.
- Educate and instruct patients, family members, or other patient representatives on rehabilitative care and activities necessary to promote health, safety, and independent living.
- Continually assess and revise the occupational therapy care plan, participating with nursing in the multidisciplinary care plan as appropriate.
