Job Responsibilities:
- Making home visits to clients in designated geographic territories.
- Assisting the team in understanding social and emotional factors related to clients' health problems.
- Providing clinical social evaluations and planning interventions based on evaluation findings.
- Identifying community resource referrals to address clients' practical and environmental needs.
To be successful in this role, you will have a Master's degree in Social Work, licensure as a Licensed Clinical Social Worker, and at least 2 years of community-based experience. You will also possess strong interpersonal skills, ability to work independently, and solid computer skills.
