Job Description:
BAYADA Home Health Care is seeking a per diem / prn Home Health Physical Therapist for an opportunity performing home health visits in Cape May County, New Jersey.
This role involves making home visits to clients in designated geographic territories, performing diagnostic tests and measurements, developing and implementing individualized care plans, and educating clients and family members on rehabilitative care and activities necessary to promote their health and safety.
The ideal candidate will have a current New Jersey Physical Therapist license, a minimum of one year of recent work experience as a Physical Therapist, and strong interpersonal, organizational, and problem-solving skills.
Responsibilities:
- Make home visits to clients in designated geographic territories
- Perform diagnostic tests and measurements
- Develop and implement individualized care plans
- Educate clients and family members on rehabilitative care and activities
- Accurately document observations, interventions, and evaluations pertaining to client care management and services provided
Qualifications:
- Current New Jersey Physical Therapist license
- Minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
- Strong interpersonal, organizational, and problem-solving skills
