Responsibilities:
- Make home visits to assess, diagnose, and provide therapeutic interventions for clients with speech, language, communication, or swallowing difficulties.
- Develop and implement individualized treatment plans based on client needs and goals.
- Educate patients, family members, or caregivers on rehabilitative care and activities necessary to promote independent living.
- Accurately document observations, interventions, and evaluations using electronic medical records (EMR).
Qualifications:
- Certification as a Speech Language Pathologist from the American Speech-Language-Hearing Association (ASHA) or equivalent experience.
- Prior experience working with older adults or individuals with disabilities preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
