We are looking for an experienced Speech Language Pathologist to perform home health visits for our office located in Allentown, PA. The ideal candidate will have prior home care experience and be comfortable working with adult and geriatric patients in Senior Living Facilities.
The successful candidate will be an integral member of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients in the comfort of their own homes.
Responsibilities:
- Make home visits to assess, diagnose, and provide therapeutic interventions for clients experiencing speech, language, communication, or swallowing difficulties.
- Develop and revise treatment plans based on evaluations, physician's orders, and recommended goals.
- Educate patients, family members, or other representatives on rehabilitative care and activities necessary for independent living.
- Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
Qualifications:
- A current license as a Speech Language Pathologist in Pennsylvania.
- Possess a certificate of clinical competence in speech from the American Speech and Hearing Association, or equivalent educational requirements.
- Solid computer skills and prior experience with electronic medical records (EMR) preferred.
