Job Title: Per Diem / PRN Home Health Physical Therapist
About the Role
BAYADA Home Health Care is seeking a compassionate and skilled physical therapist to join our team as a per diem / prn home health physical therapist in S Burlington, Vermont.
Key Responsibilities
- Perform diagnostic tests and measurements to assess client needs
- Develop and implement individualized care plans to promote client health and safety
- Educate and instruct clients and family members on rehabilitative care and activities
- Accurately document observations, interventions, and evaluations pertaining to client care management
Requirements
- Active and current license to practice as a professional physical therapist in the state of VT
- Previous home health experience required
- Commitment to a 13-week assignment
- OASIS experience required; previous HCHB experience preferred
